MONTHLY QUALITY ASSURANCE REVIEW
Effective May 15, 2017, the Division of State Group Insurance (DSGI) will begin requesting documents as part of a monthly quality assurance review to confirm that an employee’s dependents are eligible for insurance coverage under the pre-tax benefits program. This new process will apply to new hires, marriages, births and other life events occurring during the year as allowed by a qualifying status change (QSC) event. Only employees captured in a random sample of QSC events will receive the documentation request.
Employees will have 60 days from the date of the request to provide legible documentation. If the documentation is not received by the deadline date or if the documentation does not prove the eligibility of the dependent, the dependent’s insurance coverage will be terminated prospectively. The following table lists the types of eligible dependents and documents required to verify each relationship.
